What documentation do I need to host a commercial kitchen?

  1. Valid Business License
  2. Proof of last health inspection report
  3. Certificate of General Liability Insurance
  4. A photo uploaded of the last time the hood was inspected.  The sticker is on your hood.
Note*  If you are offering commissary services, the restaurateur must upload their certificate of general liability as well.
